چکیده

Background: Spontaneous isolated dissection of the posterior inferior cerebellar artery (PICA) is rare and presents with variable clinical course and prognosis. We report a case of spontaneous isolated PICA dissection presenting with lateral medullary infarction. Case Report: A 57-year-old female patient presented with right leg ataxia, headache, nausea and vomiting with a past medical history of hypertension. Neurological examination revealed right leg ataxia and left-side sensory change with right ptosis and miosis. The brain magnetic resonance imaging and magnetic resonance angiography showed an acute infarction in the right lateral medulla with abrupt luminal stenosis and subtle dilatation of the proximal segment of the right PICA. Transfemoral cerebral angiography showed an abrupt segmental stenosis of the right proximal PICA with a pre-stenotic fusiform dilatation, suggestive of dissection. The patient had a good clinical outcome with antiplatelet therapy. Conclusion: We report a rare case of spontaneous isolated PICA dissection with lateral medullary infarction. High-Resolution Contrast-Enhanced 3D-Spoiled Gradient-Recalled Imaging for Evaluation of Intracranial Vertebral Artery and Posterior Inferior Cerebellar Artery in Lateral Medullary Infarction

Although several causes of lateral medullary infarction (LMI), such as atherosclerosis, dissection and thromboembolism in the intracranial vertebral artery (ICVA) and the posterior inferior cerebellar artery (PICA) were reported, there have been few radiological studies for ICVA and PICA in LMI as yet (1, 2). Digital subtraction angiography (DSA) is a gold standard for diagnosis of arterial les...

Vertebral Artery Occlusion Following Hyperextension

Vertebral Artery Occlusion Following Hyperextension and Rotation of the Head • This is a case report of a lateral medullary syndrome of Wallenberg following occlusion of one vertebral artery and stenosis of the opposite artery precipitated by combined motion of hyperextension, rotation, and lateral flexion of the head within physiological limits. Sufficient duration of such head position appear...

Fibromuscular dysplasia of the vertebral artery presenting with lateral medullary syndrome: a case report.

We have recently encountered a rare case of fibromuscular dysplasia (FMD) of the vertebral artery (VA) presenting as lateral medullary syndrome. A 39-year-old male was admitted to our hospital due to vertigo, dysarthria and numbness of the left face and the right limbs. A magnetic resonance imaging (MRI) and magnetic resonance angiography (MRA) of brain revealed lateral medullary infarction in ...

Infarction in the territory of the medial branch of the posterior inferior cerebellar artery.

We report 10 cases of cerebellar infarction in the territory of the medial branch of the posterior inferior cerebellar artery (mPICA). Axial sections on MRI through the middle of the medulla and the cerebellum showed the infarction as a triangular area with a dorsal base and a ventral apex directed towards the fourth ventricle. The infarct also involved the lateral and dorsal medulla when the m...

Lateral Medullary Ischemic Infarct Caused by Posterior Inferior Cerebellar Artery Aneurysm

Posterior inferior cerebellar artery (PICA) aneurysms are rare. The most common complication of intracranial aneurysms is rupture causing subarachnoid hemorrhage. Ischemic infarct, although more common in giant thrombosed aneurysms, is a very rare manifestation of small intracranial aneurysms. Here we describe a patient who presented with lateral medullary acute infarction associated with an ip...
